Patient Outcomes: Results We’ve Achieved for Our Patients
Across our practice, most patients reported fewer symptoms at their latest visit than when they started care. We track every patient’s symptoms with the same standardized questionnaires used across behavioral health, for depression (PHQ-9), anxiety (GAD-7), and ADHD (ASRS).
Patients who reported fewer symptoms:
Depression
(PHQ-9)
Anxiety
(PHQ-9)
ADHD
(PHQ-9)
Note: “Fewer symptoms” means a lower score at the latest visit than at baseline (first visit). Based on paired samples of 7,257 (PHQ-9), 7,365 (GAD-7), and 7,383 (ASRS) patients. A lower score indicates improvement but is not the same as clinical remission or treatment response.
Our Collaborative Care Model (CoCM)
In addition to direct referrals, we also partner with primary care practices by providing psychiatric consultations, care coordination support, and educational resources. Whether you are launching a new CoCM program or seeking to optimize an existing one, we are here to help.
