Long-Acting Injectables (LAIs)

LAIs are an alternative to daily oral medications for patients managing conditions like schizophrenia, bipolar disorder, and psychosis. Administered by injection once every few weeks or months, they help maintain consistent medication levels, reduce the risk of relapse, and support long-term stability.

Chronic Care Management (CCM)

Our CCM program supports patients managing two or more chronic conditions — behavioral or physical — through ongoing care coordination between appointments.
