Chronic Care Management (CCM) offers specialized support for patients managing not just psychiatric conditions, but also any co-occurring physical health concerns expected to last at least 12 months. CCM is a proactive approach to care that involves ongoing communication, medication management, and seamless collaboration between all your healthcare providers for better health outcomes.

Who is Eligible for CCM?

CCM is designed for patients who are navigating the complexities of two or more chronic health conditions requiring ongoing care coordination. To ensure you can access this valuable service, your insurance plan needs to cover the Chronic Care Management Model. We also accept self-pay patients and those with out-of-network benefits. If your plan offers out-of-network coverage and you’ve confirmed eligibility with your insurer, we can either submit claims on your behalf or provide you with a superbill for reimbursement.

Yes, we are accepting new patients. We can see patients from age 5 years and older.

TMS works by using short, magnetic impulses to stimulate underactive nerve cells in brain regions known to regulate mood, helping to reset brain chemistry.

When you’re on the main page and logged in to your patient portal, look for the “Upload Docs” button in the purple banner at the top of the page. It’s the last option to the right. Click on it, and you will then see the option to upload documents and/or a picture of the front and back of your primary and/or secondary insurance cards.
