This program provides behavioral health care needs in the primary care setting:

  • Increasing follow-up encounters utilizing an experienced and dedicated behavioral health care manager.
  • Ensuring the highest quality of behavioral health care with the consultation of expert psychiatric practitioners. 
  • Proven in studies to improve behavioral and overall health.

What should I expect when enrolled in the ASP-CoCM program?

  • You will engage with your primary care provider for treatment and the behavioral health care manager for follow ups.
  • You may engage with a psychiatric specialist when needed, but this is not required as part of the program.
  • There will be several check in points to make sure that treatment is effective and to make changes to the treatment plan when needed.
  • Enrollment in the program varies in length depending on the response to treatment.
